1.  Certificate of Local Government of Origin
2. . Birth Certificate or Declaration of Age.
3.  Online teller for the purchase of Post-UTME Application Form
4. Two copies of your on-line Application Form
5. Five (5) recent passport photographs.
6.  Photocopy of UTME Score slip.
7. Examination result to include: WAEC, NECO, NABTEB etc
